Salary Range: £43,954 - £52,932, with a BAR of £54,643 - £59,343 per annum

Sanctuary Personnel is seeking a skilled Senior Home Care Team Leader to join an esteemed team within Adult Community Care Services.

The successful candidate will collaborate closely with the Social Care Team Co-ordinator within an integrated health and social care adult community team. As a pivotal member, the Team Leader will oversee the daily delivery of high standards of assessment, planning, implementation, and evaluation of care programs for a designated group of service users within the Home Care Service. Responsibilities include providing personal care and basic health care tailored to assessed needs, facilitating independent and safe living within individuals' homes. Ensuring continuity, consistency, and equitable provision of care within the community is integral to this role.

Requirements:

  • Registered health or social care professional
  • Evidence of CPD at diploma level
  • Health and Social Care Degree
  • Teaching / mentoring qualification
  • Diploma in related field
  • NVQ A1 assessors award
  • A commitment to, and understanding of, delivering person centred care practice
  • Work experience relevant to the post advertised
  • Demonstrable excellent administrative and computer based skills
  • The ability to respond appropriately and be resilient in various situations, that may sometimes cause conditions of pressure and emotional stress
  • A commitment to personal development and actively to take advantage of training opportunities
  • Demonstrable understanding and experience of participation within the VQ process
  • Proficient in the use of the full range of Microsoft product and the ability to learn new systems related to the role
  • Ability to work autonomously, manage own time effectively and take decisions
  • A flexible approach to working
  • Ability to multi-task
  • Understanding and knowledge of safeguarding process 
  • Previous proven managerial / supervisory experience and the ability to lead and support staff
