About the Role

We are currently seeking experienced Nurses to join our well-established team as Community Clinical Nurse Specialists in Palliative Care. In this role, you will directly assess, support, and advocate for patients and their carers facing complex, distressing, and life-limiting illnesses. Your responsibilities will include evaluating referrals for admission to our service, conducting comprehensive assessments of patients requiring sensitive and complex palliative care, and delivering holistic specialist care tailored to individual needs, preferences, and cultural backgrounds. This involves clinical assessment, care planning, progress monitoring, requesting relevant investigations, and taking appropriate actions based on results.

About You

We are looking for Registered Nurses with a background in palliative care and evidence of ongoing professional development in this field. Excellent communication skills are essential, along with qualifications in advanced clinical assessment and Mentorship. Non-medical prescribing qualifications are a prerequisite for this position.
