About the Role

We are delighted to invite applications from seasoned nurses to join our client's well-established and friendly team as a Clinical Nurse Specialist in Reading. In this role, you will provide support to patients with an End-of-Life diagnosis to enable them to remain in their homes. This position primarily entails telephone triage responsibilities.

Key duties include assessing referrals for admission to the service, conducting comprehensive patient assessments, planning, implementing, and evaluating care delivery according to evolving healthcare needs, identifying actual or potential palliative care emergencies, formulating action plans to alleviate distress, ensuring collaborative development, documentation, and communication of such plans with other professionals, patients, and their families. Additionally:

  • Utilising advanced assessment skills to identify reversible symptoms, devise a care plan, and implement it
  • Offering holistic specialist care tailored to patients' needs, preferences, and cultural background. This encompasses clinical assessment, care planning, progress monitoring, requesting relevant investigations, and acting upon results
  • Establishing and maintaining effective communication, often of a highly complex and sensitive nature, with patients, carers/relatives, and professionals across health and social services. This involves employing verbal and written communication and negotiation skills to overcome barriers to understanding with various stakeholders.

About You

We seek a Registered Nurse with experience as a community palliative care nurse, or alternatively, a nurse with palliative care experience and evidence of ongoing professional development in this field. Strong communication skills are essential, along with qualifications in advanced clinical assessment, mentorship, and non-medical prescribing, or a willingness to undergo training.
