Main Duties and Responsibilities:

Clinical Responsibilities

  • Conduct comprehensive assessments of patients/clients with complex care needs, including those with undiagnosed conditions.
  • Develop, implement, and evaluate tailored care programs to address these needs.
  • Provide guidance and direction to fellow staff in assessing, planning, implementing, and evaluating patient/client care.
  • Initiate risk assessment procedures to identify and mitigate potential health risks for patients/clients with complex needs.
  • Lead teams in emergency and acute situations, ensuring patient/client needs are met and support is provided to significant others.
  • Utilise diverse sources of information to identify patient problems and inform decision-making processes, including the initiation of clinical tests within the field of expertise.
  • Encourage and incorporate service user involvement in care planning, delivery, and evaluation, while respecting their preferences, beliefs, and dignity.
  • Independently communicate effectively with multidisciplinary teams and other involved agencies.

Training and Education

  • Champion the promotion of health and well-being for clients/patients and their significant others, integrating health promotion into care planning and delivery.
  • Act as a supervisor for staff within the designated area, including mentoring and supporting students and other learners.
  • Foster a conducive environment for quality learning and assessment, developing educational programs to meet staff needs.
  • Maintain personal continuing professional development, working towards achieving graduate status.
  • Develop and uphold competence in additional skills relevant to patient/client needs.
  • Participate in annual mandatory training sessions.

Leadership/Managerial

  • Contribute to team recruitment and retention efforts.
  • Coordinate team activities, ensuring clear communication systems and addressing work-related issues.
  • Ensure team awareness of organizational objectives, policies, and procedures.
  • Conduct professional development reviews and support staff with performance issues.
  • Serve as a role model for junior staff, promoting high standards of practice and professional values.
  • Assist with shift planning for the practice area.

Communication

  • Communicate effectively with patients/clients and their significant others, considering cultural backgrounds and cognitive abilities.
  • Maintain clear communication with multidisciplinary team members, showing sensitivity to cultural and language differences.
  • Manage conflicts, advocating for patients/clients, significant others, and junior staff.
  • Ensure written communication meets organisational and professional standards.

Audit/Information Technology/Research Activity

  • Collaborate with audit departments to conduct clinical audits and uphold practice standards.
  • Utilise incident reporting systems to document risks to patient care quality.
  • Promote evidence-based practice among staff.
  • Lead in specific practice aspects, integrating new initiatives into staff practices.
  • Access relevant organisational electronic information systems with appropriate training.

Essential

  • Current registration with relevant professional body.
  • Evidence of Continuing Professional Development at diploma level.
  • Post-registration experience.

Desirable

  • Completion of Preparation for Mentors course.
  • Diploma in a related field.
  • Level 3 Certificate in Assessing Vocational Achievement or equivalent.
  • Portfolio evidence meeting agreed competence criteria.
  • 'A' unit or Assessors course completion.
  • Completion of an approved mentor preparation course or equivalent education certificate.
