Job Summary:

We are seeking a Staff Nurse to join our Adult Community Services team. In this role, you will contribute to assessing, planning, implementing, and evaluating care programs for patients within a defined caseload. Additionally, you will assist in caseload management, provide high-quality patient care, and participate in the supervision and teaching of staff and student nurses.

Clinical Responsibilities:

  • Conduct assessments and develop care programs for patients in their homes.
  • Provide nursing care as prescribed, demonstrating autonomy and expertise.
  • Guide and support other staff in delivering care programs.
  • Promote patient involvement in care planning and decision-making.
  • Empower patients to enhance their independence and quality of life.
  • Collaborate effectively with multidisciplinary teams and external agencies involved in patient care.

Training and Education:

  • Act as a mentor and supervisor to students and learners.
  • Foster a conducive learning environment and participate in reflective practice.
  • Maintain personal professional development and competence in relevant skills.
  • Engage in mandatory training and educational initiatives.

Leadership / Managerial:

  • Assist in caseload management and task delegation in coordination with team leaders.
  • Adhere to organisational policies and procedures, setting a professional example.
  • Serve as a role model for junior staff, promoting excellence in practice and values.

Communication:

  • Communicate effectively with patients and their families, considering cultural and cognitive factors.
  • Ensure clear communication within the multidisciplinary team, respecting diversity.
  • Advocate for patients, families, and colleagues, ensuring their voices are heard.
  • Maintain documentation standards in accordance with organisational and professional guidelines.

Audit / Information Technology / Research Activity:

  • Participate in clinical audits to uphold practice standards.
  • Utilise reporting systems to address risks to patient care quality.
  • Promote evidence-based practice in patient care.
  • Access relevant organisational electronic information systems with appropriate training.

Professional Requirements:

  • Hold effective registration with the Nursing & Midwifery Council (NMC).
  • Fulfill Revalidation requirements as per NMC standards.
  • Support colleagues in meeting Revalidation requirements as appropriate.
  • Proactively manage Revalidation processes and seek support when needed.

Health and Safety:

  • Adapt to varying and potentially hazardous work environments.
  • Employ appropriate risk assessment tools to ensure personal and others' safety.

Other:

  • Adhere to Health and Safety at Work regulations and principles of data protection.

Key Criteria: Essential:

  • Registered Nurse.
  • Teaching/mentoring qualification.
  • Community nursing experience.
  • Valid driving license.
  • Proficiency in wound care.
  • Excellent communication and writing skills.
