Sanctuary Personnel is seeking a dedicated Band 6 Senior Mental Health Practitioner to join a dynamic team, providing frontline response within Mental Health Services. This position entails assessing and supporting individuals of all ages who present at the acute hospital with Mental Health and Learning Disabilities.

Key Responsibilities:

  • Comprehensive Assessments: Take responsibility for conducting thorough assessments of individuals referred to the service, ensuring comprehensive understanding of their needs and risks.
  • Care Planning and Coordination: Develop care plans, provide support, signpost, and make onward referrals as necessary, collaborating with service users and other agencies to ensure their needs are met effectively.
  • Service Development: Actively contribute to the development of the service and professional practice, seeking innovative ways to enhance service delivery.
  • Communication and Confidentiality: Maintain sensitive, confidential, and professional communication with all stakeholders, including service users, families, and external agencies.
  • Collaboration and Liaison: Liaise with various teams within Social Care services, Police, Safeguarding, and third-sector agencies to facilitate holistic care provision.

Requirements:

Qualifications and Knowledge:

  • Registered Nurse or Allied Health Professional (Mental Health) with demonstrable experience in risk assessment, crisis management, and intervention.
  • Familiarity with legislative requirements and processes in social and health care, including the Mental Health Act and Community Care Act.
  • Proficiency in report writing and evidence-based practice.

Skills and Abilities:

  • Excellent verbal and written communication skills.
  • Ability to work effectively under pressure and in crisis situations.
  • Strong therapeutic skills, including empathy and active listening.
  • Aptitude for decision-making, negotiation, and solution-focused working.
  • Competence in using assessment tools and computer systems.

Other Requirements:

  • Completion of mandatory training such as Breakaway (personal safety).
  • Willingness to work over a range of shifts covering a 24-hour period.
  • Possession of a clean driving license.
