Location: Isle of Wight, primarily community-based with some office & outpatient clinic involvement

Working Hours: Full-time position, including some shift work & occasional lone working

Sanctuary Personnel is seeking a dedicated Senior Mental Health Practitioner to join a dynamic team providing frontline support and treatment services. This role involves working closely with individuals experiencing acute crisis to deliver comprehensive assessments and care interventions.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals referred to the service, ensuring thorough evaluation of their needs and risks.
  • Develop and implement care plans tailored to individuals requiring acute and intensive support, in collaboration with service users and relevant agencies.
  • Facilitate smooth transitions for service users exiting the program, coordinating with other services/agencies to ensure continued support.
  • Actively contribute to service development and professional practice enhancement initiatives.
  • Uphold equality and confidentiality standards in all interactions and behaviours.
  • Maintain clear and effective communication channels with service users, families, and other involved parties.

Essential Duties:

  • Efficiently gather and process complex information from various sources to facilitate timely referrals and interventions.
  • Coordinate workload effectively, ensuring safe and timely responses to all referrals.
  • Provide a range of services across different settings, prioritising alternatives to hospital admission when appropriate.
  • Conduct timely and thorough assessments of individuals' health and social needs, adhering to evidence-based practices.
  • Collaborate closely with multidisciplinary teams to deliver effective services that address holistic health and social care needs.
  • Demonstrate innovative practice and contribute to service and policy development initiatives.
  • Adhere to budget management principles and participate in resource allocation decisions.
  • Provide supervision and appraisals for non-professionally qualified staff as required.
  • Maintain accurate electronic patient records and ensure compliance with data protection regulations.
  • Actively engage in research, review, and audit activities to enhance service quality and effectiveness.

Requirements:

  • Registered Nurse or Allied Health Professional with Mental Health expertise.
  • Demonstrated experience in risk assessment, crisis management, and intervention strategies.
  • Ability to engage therapeutically with individuals and groups.
  • Knowledge of legislative frameworks and best practices in social and health care.
  • Strong communication, decision-making, and negotiation skills.
