As a Mental Health Liaison Nurse, you'll play a crucial role in promoting and advancing mental health liaison services within our hospital setting. Your responsibilities will include providing clinical leadership, ensuring seamless care transitions between hospital wards and mental health services, and offering support and education to patients, families, and staff on complex healthcare needs.

Key Responsibilities:

  • Liaising with hospital wards, Adult Social Care & Mental Health Service staff, and other relevant stakeholders to facilitate effective healthcare pathways for individuals with mental health needs admitted to the hospital.
  • Enhancing care delivery through a holistic, multi-agency approach to achieve positive outcomes.
  • Participating in multidisciplinary team meetings, case conferences, and referrals to other mental health teams.
  • Advocating for individuals' rights to equitable healthcare access and making reasonable adjustments to meet individual needs.
  • Collaborating with hospital, acute services, and GP practices to assess and address complex healthcare needs, including mental health assessments and discharge planning.
  • Providing guidance to care staff on various health needs and presentations, such as dementia and psychotic disorders.
  • Continuously updating clinical knowledge and skills to drive improvements in practice and promote equality and diversity in healthcare access.

Qualifications and Skills:

  • Registered Mental Health Nurse (RMN) with NMC registration.
  • Mandatory registration with Jersey Care Commission upon appointment and yearly maintenance.
  • Professional degree.
  • Proficiency in mental health-related procedures and interventions.
  • Familiarity with local and national policies, including Safeguarding and Mental Health Law.
  • Ability to conduct mental health and risk assessments using evidence-based tools.
  • Strong communication, report writing, and IT skills.
  • Adaptability and flexibility to meet the evolving needs of the liaison team.
  • Experience in coordinating clients within a caseload and working independently.
  • Knowledge of clinical governance and audit processes.

Experience and Personal Attributes:

  • Demonstrated experience and proficiency in applying skills post-qualification.
  • Registration with the Nursing and Midwifery Council.
  • Ability to work effectively within multidisciplinary teams.
  • Flexibility, emotional resilience, objectivity, and fairness in approach.
