Job Purpose:

As a Hospital Liaison Nurse specialising in Learning Disabilities, your role will involve providing clinical leadership and support to enhance healthcare provision for individuals with learning disabilities within hospital and primary care settings. Your responsibilities will include coordinating care pathways, facilitating multidisciplinary collaboration, and advocating for equitable healthcare access.

Key Responsibilities:

  • Collaborate with healthcare professionals across various services to ensure effective healthcare pathways for individuals with learning disabilities.
  • Promote a holistic approach to care delivery through multi-agency partnerships.
  • Organise and participate in various meetings and conferences to facilitate comprehensive care planning.
  • Advocate for the rights of individuals with learning disabilities and ensure reasonable adjustments are made to meet their needs.
  • Contribute to the development and review of policies to address the healthcare needs of individuals with learning disabilities.
  • Liaise with hospital and primary care services to ensure proper identification and recording of individuals with learning disabilities.
  • Provide guidance and training to healthcare staff on addressing complex health needs related to learning disabilities.
  • Stay updated on clinical best practices and contribute to continuous professional development initiatives.

Qualifications:

Essential:

  • First Level Registered Nurse (RNLD/RNMH/RNID)
  • Teaching/Mentorship qualification
  • Professional degree

Desirable:

  • Minimum of 2 years post-qualification experience, including 1 year in a relevant specialty area
  • Relevant post-qualification speciality award or evidence of professional development
  • Working towards 120 credits at Level 7

Knowledge:

Essential:

  • Proficient in a range of health-related procedures supported by theoretical knowledge
  • Familiarity with relevant local and national policies, including Safeguarding, Disability Law, and Capacity and Self-Determination Law (2016)
  • Skilled in assessing individual health needs and coordinating care packages

Desirable:

  • Understanding of behavioural methodology such as Positive Behaviour Support

Technical Work Based Skills:

Essential:

  • Possession of a driving licence and own vehicle
  • Proficiency in work-based skills (e.g., Dementia care, syndrome-specific care, catheter care), or willingness to develop these skills on the job

Desirable:

  • Proficiency in a second language
