Job Title: Band 6 Senior Mental Health Practitioner (Nurse/Allied Health Professional)

Salary: £35,390 - £42,618 per annum
Sanctuary Personnel are seeking a dedicated Mental Health Practitioner to join our client's frontline response team within Mental Health Services. This position offers an opportunity to work with individuals of all ages referred for support, providing assessments and care primarily within acute hospital settings.
Duties:
- Conduct comprehensive assessments to determine the needs and risks of individuals referred to the service.
- Develop and implement care plans, providing support, signposting, and onward referrals as necessary.
- Collaborate with service users and other agencies to ensure their needs are met effectively.
- Actively contribute to the development of the service and professional practice.
- Maintain professionalism in all interactions, adhering to equality principles and ensuring confidentiality.
- Liaise with various stakeholders including other healthcare teams, social care services, police, safeguarding, and third-sector agencies.
Communication and Relationships:
- Efficiently gather and process information from various sources regarding referrals.
- Ensure clear communication of interventions and plans to service recipients, families/carers, and other involved agencies.
Analysis and Judgement:
- Plan and organise workload effectively, ensuring safe and timely responses to all referrals.
Planning and Organising:
- Provide a range of services in different settings, aiming to offer alternatives to hospital admission where suitable.
- Identify the need for referrals to safeguarding services when necessary.
Patient and Client Care:
- Conduct timely assessments considering health and social wellbeing.
- Formulate evidence-based care plans tailored to individual needs.
- Collaborate closely with other agencies and care providers to ensure comprehensive support.
Service and Policy Development:
- Demonstrate innovative practice and contribute to service development.
- Work collaboratively with statutory and non-statutory organisations to enhance capacity in managing acute social and health care needs.
- Participate in team development activities and training.
Managing Financial Resources:
- Ensure appropriate use of allocated resources.
People Management and Development:
- Provide supervision to non-professionally qualified staff.
- Participate in appraisal schemes and knowledge-sharing with colleagues.
Information System Use and Management:
- Effectively utilise electronic patient records and databases.
- Maintain timely and accurate data entry to ensure robust data quality.
Involvement in Surveys and Research:
- Actively review interventions and engage in practice review and audit.
- Contribute to Clinical Governance/Best Value initiatives.
Freedom to Act and Autonomy:
- Ensure completion and distribution of relevant documents.
- Undertake lone working assessments and provide appropriate support in acute hospital settings.
- Adhere to statutory responsibilities and professional regulations.
General Compliance:
- Adhere to policies and procedures, including risk management, health and safety, confidentiality, etc.
- Participate in infection prevention and control training.
- Work towards the Knowledge and Skills Framework requirements.
- Demonstrate the values and vision of the Trust in daily practice.
- Ensure compliance with safeguarding legislation and policies.
Key Skills Required:
- RN or Allied health professional (Mental Health)
- Evidence based and experience of reflective practice
- Physical skills, including assessment and communication.
- Aptitudes such as ability to work under pressure and therapeutic skills.
- 'Can do' attitude, flexibility, and adaptability.
- Effective communication skills in various settings.
- Experience in therapeutic engagement to patients in groups
- Wide knowledge/ appreciation of a range of models of practice in social and health care
