Job Purpose:

Join our Integrated Community Nursing Team as a Senior Community Nurse, delivering exceptional nursing care for patients in the community. As a vital member of the team, your role encompasses promoting a positive image of community nursing, providing nursing care, and undertaking an educational role for patients, carers, and team members.

Key Responsibilities:

Communication and Relationships:

  • Maintain confidentiality in compliance with the Data Protection Act and organisational policies.
  • Accurately manage records, audits, and statistics, adhering to specified timelines.
  • Communicate effectively with relatives/carers, offering necessary support and guidance.
  • Prepare and lead team meetings in the absence of the Team Leader, facilitating the exchange of patient reports and relevant information.
  • Participate in professional meetings/groups and attend Locality Multidisciplinary Team meetings.
  • Liaise with healthcare professionals and services to plan and implement patient care.
  • Ensure effective communication systems support care programs and pathways.

Analysis and Judgement:

  • Address the need for nursing aid or equipment, notifying relevant authorities when necessary.
  • Evaluate and record the effectiveness of care episodes and discharge processes.
  • Participate in the development of collaborative and innovative practices within the Integrated Community Nursing Team.

Planning and Organising:

  • Plan, prioritise, and delegate individual care needs in the absence of the Team Leader, reflecting available staffing levels.

Patient and Client Care:

  • Undertake assessments, plan care, and identify health and social needs.
  • Support individuals in End of Life care and participate in on-call arrangements.

Service and Policy Development:

  • Adhere to policies, clinical guidelines, and care pathways.
  • Contribute to the educational development of the Integrated Community Nursing team.

Managing Financial Resources:

  • Promote a cost-effective and efficient service.

People Management and Development:

  • Develop and review competencies to support clinical practice.
  • Adhere to the NMC Code of Conduct and engage in professional development.
  • Assist the Team Leader with managerial duties as delegated and agreed.

Information System Use and Management:

  • Effectively use IT systems for recording patient data and communication.
  • Support the implementation of SystmOne and act as a Champion User.

Involvement in Surveys and Research:

  • Participate in clinical/audit data collection and contribute to evidence-based practice.
  • Ensure adherence to performance targets.

Key Skills Required:

Essential:

  • Registered nurse with current NMC registration.
  • Recent experience in community nursing.
  • Willingness to undertake study at Level 6 or 7.
  • Understanding of the political factors influencing community nursing services.
  • Clinically up-to-date.
  • Knowledge of manual handling and bloodborne diseases.
  • Knowledge of the Continuing Health Care (CHC) process.

 

Desirable:

  • Develop oneself and contribute to the development of others.
  • Enable people to learn and develop.
  • Have undertaken appropriate training and become a CHC case manager.
