Sanctuary Personnel is currently seeking a dedicated Community Nurse to join our client's Integrated Community Nursing Team. In this role, you will provide essential nursing care to patients within the community while upholding the highest standards of professionalism and patient support. Key Responsibilities:

  • Promoting Positive Image: Serve as a professional ambassador for community nursing, fostering a positive perception of our services within the community.
  • Patient Care: Deliver comprehensive nursing care to patients residing in the community, ensuring their physical and emotional well-being.
  • Educational Role: Educate patients, their caregivers, and new team members, contributing to the enhancement of knowledge and skills within the community nursing team.

Responsibilities:

Communication and Relationships:

  • Maintain confidentiality and adhere to legal requirements regarding data protection.
  • Accurately document patient information and participate in team meetings and professional groups as required.

Analysis and Judgement:

  • Identify service gaps and collaborate with the District Nurse Team Leader to address them effectively.

Planning and Organising:

  • Safeguard the welfare of staff, patients, and caregivers, adhering to health and safety legislation.
  • Participate in the assessment, implementation, and evaluation of patient care plans.

Patient and Client Care:

  • Assist in care assessments and ensure proper documentation of patient records.
  • Facilitate patient education and participate in staff training programs as directed.

Service and Policy Development:

  • Adhere to policies, protocols, and clinical guidelines to ensure quality care delivery.

Managing Financial Resources:

  • Promote cost-effective practices and efficient service delivery.

People Management and Development:

  • Support induction programs for new team members and contribute to community experience packages for other professionals.

Managing Self:

  • Maintain professional registration and adhere to the NMC Code of Professional Conduct.
  • Engage in personal development activities and participate in supervision and appraisal processes.

Information System Use and Management:

  • Utilise IT systems effectively for patient records and communication purposes.

Involvement in Surveys and Research:

  • Participate in nursing audits, research projects, and educational activities to enhance patient care.

General Compliance:

  • Comply with policies and procedures, including risk management, confidentiality, and infection control.
  • Uphold the values and vision, prioritising patient care and safety in all actions.

Requirements:

  • Registered Nurse with current NMC registration.
  • Awareness of current issues in community nursing.
  • Ability to maintain NMC registration and participate in mandatory training and development activities.
  • Car driver with clean licence.

  • Professionalism, flexibility, and good interpersonal skills.
  • Ability to adapt to change and support learning and development initiatives.

Communication Skills:

  • Strong communication abilities, including handling difficult situations with sensitivity.
