Job Purpose:

Join our Integrated Community Nursing Team and contribute to providing exceptional nursing care for community patients. As a Community Nurse, you will play a crucial role in promoting a positive image of community nursing, delivering patient care, and educating patients, carers, and new team members.

Key Responsibilities:

Communication and Relationships:

  • Maintain confidentiality in accordance with legal requirements and policies.
  • Accurately manage written and electronic records, submitting data and claim forms as needed.
  • Communicate with relatives/carers, offering support in collaboration with the District Nurse Team Leader.
  • Participate in team and professional meetings/groups.

Analysis and Judgement:

  • Identify service provision shortfalls and report them to the District Nurse Team Leader.

Planning and Organising:

  • Safeguard the welfare of staff, patients, and carers, adhering to health and safety legislation.
  • Participate in the assessment, implementation, and evaluation of care within the Integrated Locality Service.

Patient and Client Care:

  • Initiate/participate in care assessments as part of an Integrated Locality Service.
  • Ensure accurate maintenance of patient records and provide necessary nursing aids or equipment.
  • Assist in the assessment of Continuing Health Care under the direction of the Case Manager.

Service and Policy Development:

  • Adhere to policies, protocols, clinical guidelines, and care pathways at all times.

Managing Financial Resources:

  • Promote a cost-effective and efficient service.

People Management and Development:

  • Participate in induction programs for new team members.
  • Contribute to community experience packages for professionals seeking insight into community nursing.

Managing Self:

  • Maintain NMC registration and adhere to the NMC Code of Professional Conduct.
  • Participate in personal clinical supervision and annual appraisal with the District Nurse Team Leader.

Information System use and management:

  • Effectively use IT systems, including SystmOne, for recording patient data and communication.

Involvement in Surveys and Research:

  • Participate in nursing audits, research, and other projects directed by the District Nurse Team Leader.
  • Contribute to education audits as required

General Compliance:

  • Comply with Policies and Procedures, including risk management, health and safety, confidentiality, data quality, freedom of information, equality diversity and inclusion, and promoting dignity at work.

Requirements (Essential):

  1. Registered nurse with current NMC registration.
  2. Awareness of current issues in community nursing.
  3. Clinically up-to-date.
  4. Knowledge of the principles of manual handling and blood-borne diseases.

Physical Skills:

  1. Physically capable of performing the role and responsibilities expected of the post-holder, e.g., car driver, lone working.
  2. Car driver with a clean license.
  3. Awareness of personal safety.
  4. Attend annual update and complete e-learning modules for personal safety.
  5. Contribute to promoting health and wellbeing.
  6. Assist in the assessment of people’s health and wellbeing needs.
