Sanctuary Personnel is currently looking for an experienced Band 6 Community Mental Health Nurse (CPN) to take on a new ongoing contract role. 

Location: Stowmarket

Pay Rate: £24.58 - £25.58 per hour (assignment rate)

Contract Type: Locum, 3-month contract

Shift Pattern: Full time

Duties:

  • Assessing service users over the age of 75 with dementia and complexities in later life
  • Working closely with carers and family in ensuring care needs are met
  • Completing combined assessments, recovery plans and physical health forms as required
  • Attending the MDT with completed assessments to discuss medications and treatment options
  • Liaising with Social Services and working closely together to meet the needs of the service user. Attending all safety huddles and other  meetings as part of the team
  • Filling in as Duty Worker for the team if required
  • Care Coordination of caseload
  • Attending regular clinical supervision and Line Management Supervision
  • Offering both clinic and home visits as required

Requirements of the Community Mental Health Nurse:

  • Degree or equivalent in Mental Health Nursing
  • NMC Registration
  • Must be able to drive and have a car
  • Must have experience in assessing complex mental and physical health issues
  • Must have experience in completing Recovery Plans
  • Knowledge of working with over 75 would be welcome
  • Good IT skills required
  • Must be able to work office hours Monday through Friday
  • Good telephone manner and have a good working knowledge of IT skills
