Sanctuary Personnel is seeking Virtual Ward Nurses to join a passionate team in Hanley, Staffordshire. This role involves coordinating care for Virtual Ward patients using both home visits and digital monitoring systems.

Key Responsibilities:
- Work alongside Virtual Ward Matrons and Nurse Practitioners to coordinate patient care.
- Conduct clinical assessments and manage patient care plans, including acute cases and early discharge from hospital.
- Collaborate with healthcare and social services for joint care planning and support during patient stays.
- Provide guidance to colleagues and maintainaccurate patient documentation.
- Lead change initiatives, conduct staff training, and delegate tasks effectively.
- Ensure compliance with NMC standards and Trust policies.
- Address patient concerns and contribute to service improvement efforts.
Requirements:
- Registered Nurse with NMC registration.
- Nursing Degree or willingness to pursue further education.
- Mentorship or teaching Certificate.
- Up-to-date Life Support Certificate.
- Commitment to ongoing professional development.
- Excellent communication and teamwork skills.
- Ability to manage workload effectively and work under pressure.
- Experience in clinical assessment and patient care coordination preferred.
