Job Title: Virtual Ward Nurse 

Location: Hanley, Staffordshire 

Pay Rate: £26.97 - £40 per hour (dependent on experience and qualifications) 

Sanctuary Personnel is seeking Virtual Ward Nurses to join a passionate team in Hanley, Staffordshire. This role involves coordinating care for Virtual Ward patients using both home visits and digital monitoring systems.

Key Responsibilities: 

  • Work alongside Virtual Ward Matrons and Nurse Practitioners to coordinate patient care. 
  • Conduct clinical assessments and manage patient care plans, including acute cases and early discharge from hospital.
  • Collaborate with healthcare and social services for joint care planning and support during patient stays.
  • Provide guidance to colleagues and maintainaccurate patient documentation.
  • Lead change initiatives, conduct staff training, and delegate tasks effectively.
  • Ensure compliance with NMC standards and Trust policies.
  • Address patient concerns and contribute to service improvement efforts.

Requirements: 

  • Registered Nurse with NMC registration. 
  • Nursing Degree or willingness to pursue further education. 
  • Mentorship or teaching Certificate. 
  • Up-to-date Life Support Certificate. 
  • Commitment to ongoing professional development. 
  • Excellent communication and teamwork skills.
  • Ability to manage workload effectively and work under pressure.
  • Experience in clinical assessment and patient care coordination preferred.
