Sanctuary Social Care is currently looking for a Support Coordinator to work full-time within an Adults Team based in Kingston upon Thames. The ideal candidates will possess highly developed specialist knowledge of social work practice in Adults' Service.
The pay rate for this locum Support Coordinator job is £20 per hour (Assignment rate) and is an initial 3-month contract.
Duties:
- To be responsible for undertaking and supporting a range of case work of customers with non-complex needs under manager supervision.
- To be responsible for accepting referrals, undertaking screening and where required, providing information and advice and carrying out initial assessments.
- To apply Care Act (2014) eligibility criteria based reasoning and differentiate between customers' essential and desirable needs as part of assessment.
- To work within the statutory framework under community care.
- To always adhere to and provide feedback on the council's policies and procedures for assessment, care management and occupational therapy.
- Obtain financial information from the customer to enable a financial assessment to take place
Requirements of the Support Coordinator:
- NVQ Level 2 or equivalent
- Access to a means of transport to undertake required visits to customers.
- Experience of using case based ICT systems.
- Strong literacy, numeracy, interpersonal and IT skills.
- Experience of working within a duty provision system.
- Knowledge and experience of working in an adults social care environment within a Local Authority setting.
- Experience of assessment and care management within a Local Authority setting.
- Experience of commissioning and coordinating delivery of services.
