Purpose of the Job:

As an experienced Social Worker, your mission will be to listen, connect, and empower adults and carers to promote their well-being, maximise independence, and access community resources. You'll work intensely to help individuals regain control during crises and support them in planning for the future. Through meaningful conversations, assessments, and support plans, you'll facilitate individuals and their careers in building fulfilling lives within their communities.

Main Activities & Responsibilities:

  • Utilise a strength-based approach to support adults with complex needs, ensuring intensive support to reduce and manage risks.
  • Collaborate with other professionals, fostering a multi-agency approach to enable independence.
  • Ensure compliance with statutory requirements, including best practices under the Care Act 2014 and related legislation.
  • Provide autonomy in applying relevant legislation, policies, procedures, and social work theory.
  • Support professional and practice development within teams, offering reflective practice, peer support, mentoring, and a 'buddy' system.
  • Identify and support individuals and carers at risk of abuse or neglect, utilising Making Safeguarding Personal principles.
  • Represent the Authority in court proceedings and contribute to decision-making processes.
  • Engage in meaningful conversations to determine eligibility under the Care Act (2014) and identify available resources.
  • Build effective relationships with agencies and the community to signpost appropriately.

Requirements:

  • Professional qualification in social work.
  • Current registration with Social Work England.
  • Significant experience in social work, including assessment and support planning.
  • Extensive experience in social care with adults, especially older adults, and those with physical disabilities.
  • Demonstrated experience in collaborative working with health and social care agencies.
  • Experience in utilising community resources to create sustainable care packages.
  • Ability to travel throughout the county, to families, young people, internal and external clients and within a timely manner.
