Purpose of the role:

As a Social Worker, your aim is to actively listen, build connections, and empower adults and carers to enhance their well-being, achieve greater independence, and utilise community resources effectively. You'll provide dedicated support during challenging times, assisting individuals in regaining control and planning. Utilising meaningful discussions, assessments, and personalised support plans, you'll guide individuals and their caregivers in creating fulfilling lives within their communities.

Key Responsibilities:

  • Work with adults, using a strength-based approach, to address diverse care needs and manage risks.
  • Collaborate with professionals for a multi-agency approach, promoting independence.
  • Meet statutory obligations under Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
  • Participate in reflective practice, offering peer support and shadowing opportunities.
  • Identify and support at-risk adults and carers, using Making Safeguarding Personal principles.
  • Represent the Authority in court proceedings and advocate for service users.
  • Conduct eligibility assessments under the Care Act (2014) through meaningful conversations.
  • Build strong relationships with agencies and the community for effective signposting.
  • Develop support plans focusing on independence and meeting Care Act (2014) requirements.
  • Gather and analyse data for informed decision-making.
  • Identify potential deprivation of liberty issues and prioritize least restrictive care.
  • Promote child welfare and discuss safeguarding concerns with relevant teams.
  • Assess eligibility for Continuing NHS Healthcare and contribute to decision-making.
  • Fulfil duty responsibilities and maintain accurate records.
  • Participate in reflective practice, supervision, and professional development.

Requirements:

  • Professional qualification in social work.
  • Current registration with Social Work England.
  • Significant experience in social work, including assessment and support planning.
  • Extensive experience in social care with adults, especially older adults, and those with physical disabilities.
  • Demonstrated experience in collaborative working with health and social care agencies.
  • Experience in utilising community resources to create sustainable care packages.
  • Ability to travel throughout the county, to families, young people, internal and external clients and within a timely manner.
