Pay Rate: £28 per hour (assignment rate)

Duties:

  • Apply the Care Act 2014 and enhance eligible people’s well-being
  • Apply their social care knowledge and skills to understand and build relationships, and work directly with individuals, their families, and carers in a strengths-based way, to enable and empower them to achieve best outcomes
  • Work within statutory frameworks to undertake assessments and reviews with adults and carers with less complex needs, where screening does not trigger the need for an assessment by a Social Worker or Occupational Therapist, and to plan care and support making the best use of available resources to enable people to have better lives
  • Work in the Hospital Service, hospital discharge team
  • Assess the social care needs of adults and carers under the Care Act 2014, in a timely way, applying the principles of good conversation, professional curiosity and motivational interviewing, using a strength- based approach
  • Determine eligibility for adult social care services
  • Write clear, concise, and thorough social care reports meeting statutory guidelines, maintain accurate and timely case records and share information appropriately
  • Support residents to maintain their independence as much as possible, by providing preventative services such as equipment, minor adaptations, care technology, digital solutions, referrals to voluntary sector partners or enablement
  • Work with adults and carers to develop and agree support plans (including in an emergency), to promote well-being by meeting eligible, unmet needs and make clear informed decisions about the level of support required
  • Ensure these plans use resources to best effect and that authorisation is sought before implementation via the Service Acquisition and Market Management Service
  • Seek advice and support from a manager whenever necessary
  • Emulate our corporate values in all interactions with residents, colleagues, and partners
  • Work together with an independent advocate if the adult needs their support to be involved, families and partners such as providers, the NHS and voluntary sector
  • Make sure residents and families are advised on our charging policy
  • Explain personal budgets clearly to adults and carers and promote direct payments
  • Complete person-centred reviews of the intended outcomes in the support plan and agree if anything needs to change
  • Identify if the person’s needs have changed and if a reassessment is required
  • Engage and fully participate in supervision and to apply critical reflection to improve practice
  • Engage in team meetings and team activities
  • Identify situations where adults are at risk and complete appropriate safeguarding referrals
  • Where suitable, to support the allocated Enquiry Officer with required safeguarding actions to ensure resident’s safety
  • Identify situations where an adult may have issues with capacity that will affect their decision making and refer to a manager for a Mental Capacity Assessment to be completed
  • Participate in Duty with management oversight
  • Take responsibility for personal learning and development alongside the line manager

Requirements of the Social Care Assessor:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
