Job Title: Band 6 Clinical Pharmacist

Job Purpose:

Join our team in delivering efficient and comprehensive pharmaceutical services, including clinical, dispensary, chemotherapy, medicines information, and mental health support. Emphasis will be on providing Clinical Pharmacist advice for prescribing and medicines optimisation to patients, Consultant teams, nursing, and Allied Health Professional personnel at ward level.

Key Responsibilities:

  • Participate in the provision and development of pharmaceutical services with support from senior clinical pharmacists.
  • Provide support to various services on a rotational basis, including Chemotherapy/Haematology Services, Mental Health Services, Urgent Care Services, and more.
  • Undertake medicines reconciliation and ensure adherence to Trust standards.
  • Liaise effectively with pharmacy staff, nursing staff, and other healthcare professionals to ensure clear and timely communication.
  • Provide clinical management, support, and supervision to pre-registration pharmacists, clinical placements, and undergraduate vacation pharmacy students.
  • Seize professional development opportunities, including in areas such as palliative care and pediatrics.
  • Participate in yearly performance appraisals arranged by the Lead Clinical Pharmacist.

The post holder will supervise pharmacy technicians and assistants, pre-registration pharmacists, MPharm 3rd year student placements, pharmacy vocational students, and work experience students.

Communication and Relationships:

  • Maintain effective communication with patients, primary care professionals, and multidisciplinary teams.
  • Participate in daily Clinical Pharmacy service at ward level and answer medicines-related queries.
  • Utilise electronic systems for conveying information to healthcare professionals and colleagues.

Analysis and Judgement:

  • Identify monitoring requirements and intervention problems related to medicines.
  • Individualise drug therapy to meet patient needs and provide pharmacist review for new drugs.
  • Respond to in-patient audits and support safe storage of medicines at ward level.

Planning and Organising:

  • Ensure timely medicines reconciliation and supply to in-patients.
  • Promote timely production of discharge prescriptions and provide support to various pharmacy services.

Patient and Client Care:

  • Undertake clinical pharmacist duties ensuring patient safety and medication adherence.
  • Provide advice to medical and nursing staff regarding medication choices, dosages, and administration.

Service and Policy Development:

  • Participate in departmental planning initiatives and policy development.
  • Adhere to relevant pharmacy standards and contribute to service improvements.

Managing Financial Resources:

  • Promote economic use of medicines and monitor medication expenditure.

People Management and Development:

  • Provide training and mentoring to pharmacy personnel and supervise junior staff.
  • Engage in self-directed learning and maintain professional registration.

Information System Use and Management:

  • Utilise pharmacy and electronic prescribing systems effectively.
  • Participate in surveys, research, and quality assurance activities.

General Compliance:

  • Adhere to policies and procedures, including those related to risk management, health and safety, confidentiality, and equality.

Freedom to Act and Autonomy:

  • Follow policies and procedures and contribute to patient safety.

Person Specification:

  • Masters Degree in Pharmacy and registration with GPhC.
  • Demonstrated competency in clinical issues and IT literacy.
  • Self-motivated, with excellent time management and flexibility.
  • Ability to work effectively in a team and under pressure.
  • Good communication skills and attention to detail.
  • Completion of induction programme and demonstration of competence.
  • PgD Clinical Pharmacy Practice.
  • Completion of full rotation through services with competency achievement.
