Sanctuary Health is currently looking for a Band 6 Senior Mental Health Practitioner to work in Harlow.

The pay rate for this permanent Mental Health Practitioner job is £30,401 to £39,029 per annum (depending on banding).

Job Purpose:

  • You will work as a member of the MDT in providing high quality, evidence-based , holistic assessments and care to patients in line with your professional code of conduct and Trust policies.
  • You will work within a fast paced urgent community care pathway offering an urgent and crisis response to those experiencing mental distress within the community. You will work with non-statutory and statutory services to support people while in crisis.
  • You will conduct initial CPA assessments, agree formulation of need and develop collaborative care plans underpinned by NICE Guidance delivering a range of interventions based upon individual needs.
  • As required by your manager, you will have responsibility for the day to day supervision and management of lower banded staff within your team locality. This will include the management of these staff in line with agreed Trust HR processes.
  • You will be expected to work collaboratively with key partner organisations, patients, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7- day a week service over a twenty four hour period.

Key Responsibilities:

  • Provide and oversee the management and care of patients.
  • Produce accurate and timely documentation, conduct and manage the maintenance of accurate and timely documentation / records on all service users in line with your professional code of conduct and Trust policy.
  • Oversee the conduct / contribute to the assessment process, as well as the observation and ongoing assessment, providing accurate and timely feedback.
  • Conduct risk assessment of the individual and of the potential hazards in the patients environment.
  • Be competent in use of relevant screening and assessment tools and supervise lower banded staff to undertake this function.
  • Provide comprehensive assessment, care planning, treatment and review of care in line with the requirements of the Mental Health Care Clusters.
  • Lead in the continuing development of a high quality service that is underpinned by evidence based practice, care pathways / packages and NICE and DH Guidance.

Requirements:

  • Degree or equivalent in mental health nursing
  • NMC registration
  • Experience within a similar role
