Main duties:

  • Independently assess, differentially diagnose, develop and implement specialist care plans for care home residents with swallowing needs due to acquired and progressive neurological conditions, dementia, non-neurological conditions, and frailty, as well as at the end of life.
  • Independently assess, differentially diagnose, develop and implement specialist care plans for care home residents with communication needs due to acquired and progressive neurological conditions.
  • Work with the care homes’ team members and management, as well as the wider multidisciplinary team to ensure a coordinated and holistic care plan for patients with necessary support and follow up from this care plan.
  • Provide ad hoc training and education to care home staff, to support them in their role.
  • Liaise and work closely with band 6 SLT colleague(s) from the Adult Community SLT service in ensuring a safe and equitable pathway for care home residents, from referral to discharge, across both services (DIST and Adult Community SLT service) and all south-east Essex geographical areas.
  • Be responsible and accountable for the independent prioritisation and management of own clinical caseload.

Requirements of the Speech and Language Therapist/SLT:

  • Degree or equivalent within Speech and Language Therapy and HCPC registration
  • Recent UK/NHS post qualifying experience working as a Speech and Language Therapist
