Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work within an Adults Team based in Devon. The ideal candidate will possess highly developed specialist knowledge of Social Work practice in Adult Services and be registered with the HCPC.
The pay rate for this locum Social Worker job is £31 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Duties:
- Undertake clinical assessments, triaging referrals for the team, sign posting to other services, care plans, assessing clinical risk and be able to hold/sit with risk on team case load.
- Complete/update risk assessments.
- Work with various community services.
- Creating home treatment plans with clients, and delivering this home treatment plan, liaising with inpatient wards to support/facilitate earlier discharge from inpatient services.
- The candidate would take part in MDT, professionals meetings, team core reviews, deliver supervision to Band 3 Senior Support workers.
Requirements of the Social Worker:
- Degree or equivalent in Social Work (CQSW/CSS/DipSW qualification)
- Current HCPC registration.
- Significant senior level frontline experience within Adult Services.
- Social work experience of working with vulnerable adults in local government, voluntary or independent sector.
