Duties:
- Assessment of the needs of older people
- Visits to service users in their own homes, and in care homes
- Monitoring physical health and response to medication- taking BP and pulse
- Holding a caseload of patients with organic and functional presentations under CPA and NCPA Assessing and managing risk
- Using Dialog+ where possible and Recovery Care Planning with service users and carers
- Managing your own diary and planning your day
- Attending MDT meetings and ward review meetings
- Duty working 2 half days per week
- Communicate effectively through notes on Lorenzo and letters to GPs and other involved parties
- Administering depot medication
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Must be able to drive and have access to a vehicle with business class one insurance
- Knowledge of dementia and of functional illness in older people
- Ability to liaise with social care, with carers and to signpost to carer support
- Experience of duty work an advantage- being able to respond to urgent calls
- Ability to take ECG an advantage but not essential
- Driving is essential for this role
