Duties:

  • Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as CRHTT, Adult Social Care and third sector organisations)
  • Supporting and managing a caseload undertaking face to face visits in the community setting including home visits
  • Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
  • Undertaking urgent/routine initial assessments on behalf of the team
  • Working alongside Lead Care Professionals and supporting those currently under the Care Programme Approach
  • Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm)
  • Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Experience of supporting people and their families, preferably in the community
  • Demonstrate a strong understanding of your role as a Care Coordinator/Assessor and join our dedicated multidisciplinary team working closely with a range of partners to provide appropriate community interventions for those individuals referred to the team
  • Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
  • Demonstrate effective risk assessment and risk management experience
  • Experience in the delivery of clinical interventions
  • Experience in working in team delivering person centred treatment packages
  • Knowledge of medication and the monitoring of its effects
  • Knowledge of psychosocial interventions
  • An understanding of the policies and guidelines both local and national e.g., National Institute for Clinical Excellence guidelines (Government initiative)
