As a Senior Home Care Team Leader, you will play a pivotal role within our integrated health and social care team, ensuring the delivery of exceptional care to our service users within the Home Care Service.

Job Summary:

Working closely with the Social Care Team Co-ordinator, you will oversee the daily operations of our home care service team. Your responsibilities will include assessing, planning, implementing, and evaluating care programs tailored to the needs of our service users, enabling them to live independently and safely within their own homes. You will ensure the provision of personal and basic health care, maintaining continuity and consistency in care delivery.

Key Responsibilities:

  • Manage the day-to-day operations of the home care service team, ensuring the delivery of person-centred care.
  • Provide support, supervision, and deployment of staff, ensuring coverage of rotas to meet service user needs.
  • Assist in managing and monitoring the effectiveness of care assessment, planning, and review processes.
  • Identify gaps in service provision and report them to the appropriate channels.
  • Offer social care advice to the integrated community team and other relevant stakeholders.

 

Leadership/Managerial Responsibilities:

  • Facilitate clear communication channels within the team and across the organisation.
  • Ensure staff are informed about organisational objectives, policies, and procedures.
  • Act as a professional role model, promoting high standards of practice.
  • Supervise and provide exemplary person-centred care and support.
  • Promote effective team working and manage conflicts as they arise.
  • Contribute to staff training, induction, and professional development.
  • Participate in recruitment, retention, and performance management processes.

Professional Development:

  • Identify personal educational and professional development needs.
  • Adhere to relevant codes of professional conduct and standards.
  • Proactively manage Revalidation requirements.
  • Promote evidence-based practice and maintain CPD.

Training and Education:

  • Promote health and well-being among service users and their significant others.
  • Provide supervision and mentorship to carers.
  • Plan and deliver education and training programs.
  • Maintain competence in relevant skills and undertake mandatory training.

Communication:

  • Communicate effectively with service users, their families, and multidisciplinary teams.
  • Manage conflicts and act as an advocate for all stakeholders.
  • Ensure compliance with data protection regulations and confidentiality standards.

Audit/Information Technology/Research Activity:

  • Participate in audits to monitor and maintain practice standards.
  • Utilise information systems relevant to the role.
  • Engage in service user feedback initiatives and quality improvement efforts.

Minimum Qualifications:

  • Registered health or social care professional.
  • Evidence of CPD at diploma level.
  • Health and Social Care Degree.

Full Qualifications:

  • Teaching/mentoring qualification.
  • Diploma in related field.
  • NVQ A1 assessors award.
