Banding/Grade: Band 6

Pay Rate: £26.78 - £41.12 per hour (assignment rate)

Job Type & Hours: Locum, 3-month contract, full time

Duties:

  • Provide triage assessment, clinical support, assistance and professional expertise to all health care professional referrers requiring assistance and support with Admission Avoidance
  • Offer clinical triage and support where appropriate
  • Signposting the referral caller to the appropriate service, making a direct referral to another service or offering immediate clinical advice or treatment
  • Represent the organisation in a professional and courteous manner at all times and provide advice and information using a level of clinical understanding and professional judgement
  • Work independently, exercising assessment and evaluation skills to full ability in order to avoid inappropriate hospital admissions, adjust care programmes as professionally judged to be necessary, and instigate changes in care that are evidence based and in the patient’s best interests
  • Establish and maintain excellent communication links with all Health and Social Care Professionals
  • Accept responsibility for doing a quality job, aspire to achieve, share a corporate pride in achievements and strive to act as an ambassador for good quality

Requirements of the Senior Community/District Nurse:

  • R.G.N Level 1 with current NMC registration
  • Familiarisation with and adherence to Safeguarding policies is an essential requirement of all employees as is participation in related mandatory/statutory training
  • Demonstrate a commitment to provide customer focussed services and to continuously enhance the customer experience
  • Be aware of current health and safety policies and attend all mandatory health and safety training
  • Be impartial and honest in the conduct of their official business
