Sanctuary Personnel, a committed and competitive recruitment agency is currently looking for an experienced and dedicated Band 6 Care Coordinator to fill a full-time role based in Barnet, within the Community Support and Recovery Team (CSRT).
The pay rate for this Care Coordinator job is £26 - £27 per hour (Assignment rate) and is an initial 12-month contract.
The post holder may be required to work at alternative locations within the Mental Health Services of this Trust. All requests to work flexibly will be considered.
Duties:
- Provide effective care and specialist advice for people with psychosis and complex mental health needs and their carers, in order to support their wellbeing and recovery
- Liaise with other agencies in planning and assessing programmes of care
- Provide formal, ongoing clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed
- Undertake duties designed to extend clinical and managerial expertise as delegated by the manager when required
- Deputising in the absence of the Senior Practitioner
- Carry overall responsibility for the management of a caseload, acting as a care co-ordinator, as defined by the Care Programme Approach (CPA)
- Deliver effective care and treatment that is carried out within a multidisciplinary framework and in collaboration with other agencies
- Involve service users and their carers in planning care that includes all of their individual and diverse needs
- Design individual programmes of care collaboratively with patients to maximize self management and social inclusion and reflect their own goals and aspirations
- Review and adjust plans according to changing service user needs
- Provide evidence based therapeutic interventions using developed interpersonal skills such as CBT or psycho-education
- Provide service users and their carers with clear and intelligible information which will assist them in making informed decisions about their treatment and care
- Administer medication, monitor for effects and side-effects, in accordance with NMC Standards and Trust policies
- Facilitate vocational rehabilitation and those aspects of mental health recovery concerning occupation
- Maintain clear, comprehensive and contemporaneous clinical records on all patients in response to personally generated clinical observations, in accordance with professional standards and Trust policies
- Prepare and present clinical reports as required
- Provide formal, ongoing clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed
- Undertake teaching of qualified and unqualified staff, including students
- Undertake surveys or audits as necessary to own work and participate in clinical trials/R&D activity as required
- Initiate and/or collaborate in service/clinical developments based on appropriate research evidence
- Set, monitor and ensure maintenance of standards of care and clinical outcomes
- Promote health and provide specialist advice/education regarding mental heath to professionals, service users and the public in general
- Receive regular, ongoing clinical and case management supervision; attend staff development training as required
- Undertake the duties and responsibilities of the post with due regard to confidentiality and the Trust policies on Data Protection, Equal Opportunities, Health and Safety and Smoking
- Work effectively under Service Integration with Social Care colleagues employed by the Local Authority, who will be employed under different terms and conditions of employment
- Ensure that a safe working environment and safe working practices are maintained at all times
- Practice in accordance with the infection control standard specified in the Trust policies
- Manage risk within their sphere of responsibility
- Take reasonable care of their own safety and the safety of others who may be affected by acts or omissions
- Professional responsibilities for the promotion of health, by providing up to date and relevant best practice advice, in supporting their patients/clients to live a healthy life
- Act in such a way that at all times safeguards the health and well being of children and vulnerable adults
- Comply with national legislation and local policy in respect of confidentiality and data protection
- Act at all times in accordance with the relevant professional Codes of Conduct and Accountability (including, where applicable, those for Board Members)
Requirements of the Care Coordinator:
- Degree or equivalent within Mental Health Nursing (RMN trained)
- NMC registration and a valid live pin
- Be aware of the range of health promotion information and local polices that support the promotion of health and the prevention of ill health eg. food and nutrition, stress management etc
