Main Duties: 

  • To participate fully in the multidisciplinary teams drawing on specialist knowledge/experience.
  • To contribute to service and policy development.
  • To advise line manager on service delivery, pressures and developments with regard to future needs and anticipated developments.
  • To supervise and delegate work and provide clinical support to a Speech and Language Therapy Assistant, Junior Speech and Language therapists and generic Rehabilitation Assistants.
  • To represent speech and language therapy at relevant meetings as and when required.
  • To attend and contribute to departmental team meetings and AHP network meetings.
  • To provide specialist support to adult patients/service users including support with alternative and augmentative communication aids.
  • To ensure well co-ordinated, integrated care plan for individual patients/service users.
  • To regularly liaise with charitable agencies.
  • To write detailed reports to other agencies.
  • To be able to manage the emotional consequences of working with clients with psychological, psychiatric and behavioural problems.
  • To manage and prioritise a complex speech and language therapy caseload.
  • To maintain up to date and accurate case records in line with professional standards and local Trust policies.
  • To independently assess, diagnose and treat a wide range of communication disorders for clients with complex speech, language and cognitive disorder, using both standardised assessment and informal assessment protocols.
  • To independently assess, diagnose and treat patients with dysphagia and to refer for specialist instrumental assessment of the swallow as and when indicated.
  • To be able to define a differential diagnosis based on the evidence from assessment and the multidisciplinary team, taking full account of cognitive, neuro psychological, physical and sensory deficits.
  • To continually monitor, evaluate and modify treatment plans to meet the patient’s changing physical, emotional and psychological needs.
  • To regularly liaise with and give specialist advice to professionals from other statutory agencies e.g. Social Services.
  • To contribute to interagency teams by providing specialist information and advice around managing a client’s needs with regard to a wide variety of physical, social, emotional and psychological issues to provide well co-ordinated and appropriate care plans.
  • To work flexibly across a wide variety of settings, including wards, clinics and patient’s homes providing specialist advice and management of complex patient needs.
  • To be able to manage the emotional consequences of working with terminal conditions and the effects of patients and families emotional and psychological adjustment to these conditions, using counselling skills.
  • To be alert to hazardous situations related to infection control and abuse of vulnerable adults and take appropriate action to alert the appropriate staff and agencies of potential risks to staff and patients.
  • Demonstrate good understanding of the code of good practice in relation to the Mental Health act and Mental Capacity Act.
  • Work in accordance with the safeguarding adults/children’s guidelines and the safeguarding policy at all times.
  • To manage the emotional consequences of working with highly unpleasant conditions related to client contact.
  • To devise and write specialist programmes of treatment for rehabilitation assistants, monitor and amend these as appropriate and provide support for assistants in carrying out these duties.
  • To discharge patients in a timely manner when the duty of care has been fulfilled and notify the relevant agencies.
  • To recognise own professional boundaries and be able to seek professional support from senior clinicians, and expert practitioners.
  • The prevention and control of infection is an integral part of the role of all health care personnel. Staff members, in conjunction with all relevant professionals will contribute to the prevention and control of infection through standard infection control practices and compliance with the Trust’s infection control policies in order to ensure the highest quality of care to patients.

Requirements of the Speech and Language Therapist/SLT:

  • Degree or equivalent within Speech and Language Therapy and HCPC registration
  • Recent UK/NHS post qualifying experience working as a Speech and Language Therapist
  • Experience of working with neuro rehab patients is essential
  • Experience of working with patients with dysphagia is desirable
