Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time within an Intermediate Care Team based in Southwark. The ideal candidates will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £27.27 per hour (Assignment rate) and is an initial 3-month contract.
Resposibilties:
- Completing Care Act assessments, support plans, reviews, support plans and making sure that the people of Southwark are receiving the support that meets their needs.
- Provision of a safeguarding function, to assess and investigate risk, mental capacity, safeguarding issues ensuring the use of all appropriate resources from Social Care, Health and other voluntary and statutory agencies.
- Provision of a short term intensive assessment period, assessing the person's ability to maximise their independence and to provide a robust assessment that measures the amount of ongoing support that is needed .
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
