Sanctuary Social Care is currently looking for experienced and dedicated Social Workers to work full-time in Birmingham in a Continuing Health Care Team. The ideal candidates will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rates for these locum Social Worker jobs are £24.47 per hour (Assignment rate) and is an initial 3-month contract.
The candidates will mainly be remote/home working.
Duties of the Social Workers:
- To undertake assessment/review with citizens being discharged from hospital, the reviews will be carried out jointly with CCG colleagues to determine health and social care needs and the aim will be to agree an appropriate funding stream decision - local authority, CHC, or joint funding split.
Requirements of the Social Workers:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
- Experience of working knowledge of Care Act, MCA, MHA, and CHC framework
- Have experience in, and confidence to, advocate for the person and LA in MDT decision making forums while maintaining positive working relationships with other professionals.
