Sanctuary Personnel are currently seeking an Occupational Therapist to join our client's Community Independence Service (CIS) team. In this role, you will play a crucial part in our integrated service, focusing on hospital avoidance and enhancing individuals' independence within the community.

Responsibilities:

  • Manage a caseload efficiently and effectively, ensuring timely and appropriate interventions.
  • Collaborate closely with Reablement care providers, offering updated goal plans and feedback.
  • Assess and implement moving and handling plans, incorporating Smarter Care Provision principles.
  • Undertake duty OT responsibilities on a rotational basis, including emergency visits and case cover.
  • Facilitate hospital discharge back home in alignment with the discharge to assess model.
  • Work collaboratively with other therapists and the multidisciplinary team to set and achieve joint goals.
  • Design and execute reablement programmes to assess eligibility for long-term care under the Care Act 2014.
  • Recommend and order suitable equipment for individuals' needs, providing instruction on its use and maintenance.
  • Identify and address safeguarding concerns, working alongside safeguarding adults investigating officers.
  • Prioritise interventions based on clinical reasoning and maintain high standards of evidence-based practice.
  • Screen and assess referrals, establishing priorities and initiating intervention plans.
  • Manage a diverse caseload, utilising supervision and guidance from senior staff to monitor and evaluate interventions effectively.
  • Educate individuals and their families on rehabilitation and reablement programmes.
  • Maintain accurate records of assessments, interventions, and communications in line with departmental policies.
  • Collaborate with voluntary and statutory agencies to ensure individuals have access to necessary support services.
  • Contribute to the development of departmental policies and service improvements, representing the CIS team as required.
  • Engage in continuous professional development, staying updated with current practices and participating in training and education programmes.

Requirements:

  • Qualified Occupational Therapist with relevant registration and certification.
  • Proven experience in a similar role, preferably within a community setting.
  • Strong communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
  • Sound clinical reasoning and decision-making abilities.
  • Proficiency in IT systems and willingness to undertake further training as necessary.
  • Commitment to maintaining high standards of care and professionalism.
  • Ability to adapt to changing priorities and work independently when required.
