The Role: You will be part of a multi-disciplinary team (Social Workers, Occupational Therapists, and Physiotherapists) which provides a health and social care service to the local community.

Responsibilities:

You will play a lead role in coordinating the care and support for users (and their carers/ families) pulling in the expertise of other professionals as required They will also be responsible for accurate and comprehensive inputting of case management information for monitoring and planning purposes.
