Sanctuary Personnel is seeking a dedicated Clinical Pharmacist to provide specialist support within our client's various medical settings. The successful candidate will collaborate closely with the Medical Directorate, ensuring high standards of patient care in accordance with Pharmacy, Trust, and Directorate objectives. Additionally, they will support GP Practices, exercising professional supervision over dispensing activities at a hospital in Boston, and contributing to chronic disease management.

Key Responsibilities:

  • Collaborate with the Medical Directorate to ensure patient-focused care adhering to Clinical Governance principles.
  • Review prescriptions for legality, accuracy, and suitability, and provide timely medication supply to patients.
  • Develop discharge processes to enhance efficiency and safety during patient transfer to primary care.
  • Support GP Practices with structured medication reviews and clinical medication optimisation.
  • Manage a caseload of patients with common ailments, providing appropriate interventions and referrals.
  • Contribute to multidisciplinary case conferences and provide support to healthcare teams.
  • Participate in clinical audits, drug utilisation reviews, and risk management initiatives.
  • Provide clinical training to pharmacy staff and other healthcare professionals.
  • Undertake other duties as required, ensuring compliance with relevant regulations and policies.

Qualifications and Experience:

  • Master’s degree in Pharmacy and registration with the General Pharmaceutical Council.
  • Postgraduate Clinical Diploma or equivalent experience.
  • Supervisory experience and independent prescriber qualification preferred.
  • Previous experience in hospital and primary care settings, with a strong understanding of medicines management.
  • Proficiency in computer systems and excellent communication skills.
  • Flexibility to accommodate varying work hours and travel between sites.
