Duties:

  • Be part of a multi-disciplinary team where members work closely with each other to provide an excellent service
  • Have a vital role in the team to embed social care and to deliver a high-quality service that is sensitive to the needs of our service-users and carers
  • Be responsible to the Locality Manager on a day- to- day basis for work within the Team
  • Participate in regular professional supervision with the Senior Social Worker
  • Contribute to developments in practice and service delivery within London Borough of Merton, and have a key role in building and maintaining links with other stakeholders in primary care, and the private and voluntary sector
  • Undertake holistic assessments of need and formulate care plans through multidisciplinary CPA planning process
  • Ensure that appropriate care and resources are made available to service – users with substance misuse needs, and to regularly review the care plan, and adjust services where appropriate
  • Implement Community Care Legislation Fair Access to Care Services criteria, the Mental Capacity Act, and Mental Health Act to meet service – users needs
  • Carry out risk assessments and demonstrate risk management within the context of CPA ensuring that plans are implemented and regularly reviewed
  • Support service-users to commission services they require through self – directed support and as a care coordinator arrange appropriate statutory services
  • Implement the Recovery Approach with service-users and their carers, to offer them choice and control and engage them in mental health promotion and relapse prevention work
  • Work with service-users when they are in crisis both for allocated CPA cases and to take collective responsibility for service-users open to the team
  • Write social circumstances reports on allocated service – users for Mental Health Review Tribunals and managers hearings
  • Participate in team and other duty rotas
  • Work with Carers, complete Carers assessments and implement packages of care where required
  • Liaise with other agencies both statutory and in the private and voluntary sector
  • Attend both management and professional supervision on a regular basis, and to participate actively in the supervision process
  • Attend team meetings, both clinical and business, on a regular basis

Requirements of the Adults Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
  • Demonstrate leadership in relation to the social care agenda within the team, to hold a full caseload, and complete comprehensive assessments of need, risk management and a recovery approach to service-users with substance misuse issues
  • Demonstrate effective communication with a range of agencies and professionals
  • Have sufficient knowledge and awareness of the impact of substance misuse, treatment options including medication and its associated side effects, and to be able to work with service-user concordance
  • Have a sound working knowledge of child protection and safeguarding vulnerable adults, be able to undertake investigations, write reports and follow through protection plans and close cases
