Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Adult Social Worker within the Transitions Team to work full time for Redbridge.

The salary for this permanent Social Worker job is up to £43,815 per annum.

Main duties:

  • To review the needs of service users and carers, using your ability to present ideas relating to individual need to include a range of options.
  • To facilitate service users independence and empowerment as far as possible by ensuring that at all stages of a service user’s transition plan, client and carers are kept fully informed of their rights and have access to all relevant information.
  • To ensure that individual care plans are person centred and responsive to the needs of users, including their religious, cultural, and linguistic needs, and the needs of disabled service users.
  • To sensitive to the dynamics of the client’s social and cultural situation, and facilitate communication, offering advice and appropriate level of counselling as required.
  • To attend multi-disciplinary case conferences, meetings, and reviews, as appropriate. Identifying the individuals housing needs and recommending appropriate packages of care and ensuring that service users and carers are supported in making a full contribution to meetings.
  • To understand, and commit to, principles of the Council’s Equal Access Policy, and an ability to work in a sensitive manner in the delivery of service.
  • To liaise and consult with users, carers, colleagues and with a range of statutory and voluntary agencies in compiling information to provide the basis of an assessment of client need.
  • To respond effectively and sympathetically to any complaints made about the provision of services in accordance with the appropriate Council complaints procedure.
  • Ensure the health and safety of staff and clients by complying with relevant legislation and guidance issued on safe working practices.
  • To participate fully in the development and maintenance of good liaison and cooperation with the local community, community groups, and other agencies and non-statutory organisations.
  • To attend and participate positively in multi-disciplinary team meetings as required.
  • To keep up to date with local policies and procedures and to always follow them accurately. To perform all functions of the post consistent with legislation and within the policies, plans, strategic aims, standards, and guidelines established by the Authority.
  • Any other duties appropriate to this area of work and consistent with the level of the post, as may from time to time be required.

Requirements of this role:

  • A social Work Qualification or equivalent.
  • Understanding of relevant legislation.
  • Registered or eligible for registration.
  • This post requires registration with the relevant health professional council/governing body (Social Care England).
