Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Devon, within the Hospital Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £31 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Duties of the Social Worker:
- Undertaking social care assessments/reviews and care planning with adults with severe and enduring mental health problems. All referrals are about those already supported within DPT Community Mental Health Teams.
- Completing paperwork in relation to funding applications to panel.
- Undertaking Safeguarding enquiries where relevant.
- Supporting individuals to make their own decisions and advocating for person centred provision and at the same time considering and were relevant using legal frameworks such as those available within MCA and MHA
- Assessing carer's needs
- Contributing strong social work and social care perspective to multi-disciplinary working of DPT adult mental health services - community and inpatient settings
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
