Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time within a Hospital Discharge & Locality Neighbourhood Team in Tameside. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £25.02 per hour (Assignment rate) and is an initial 3-month contract.
Your role will be undertaking D2A assessments, arranging short term packages of care, liaising and working closely with health colleagues from the Trust and working on a rota covering admission avoidance.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
- You must have relevant recent experience of working within a hospital setting and the demands and pressures of an integrated urgent care team.
- Experience of liquid logic is preferred and the ability to manage / coordinate cases.
