Key Responsibilities:
- Conduct thorough assessments of individuals and families to identify strengths, needs, and risks.
- Collaborate with service users and support networks to develop holistic care plans promoting positive outcomes.
- Offer direct support and intervention, providing practical assistance, advocacy, and emotional support.
- Empower service users, respecting their autonomy and promoting self-determination.
- Conduct risk assessments and safeguarding investigations in cases of suspected abuse or neglect.
- Take necessary actions to mitigate risks and ensure the safety of vulnerable individuals, adhering to statutory requirements and council policies.
- Work with internal and external stakeholders to coordinate support services and achieve positive outcomes.
- Participate in multi-disciplinary meetings and joint assessments to facilitate integrated service delivery.
- Maintain accurate and compliant case records, adhering to data protection regulations.
- Prepare clear and concise reports and documentation for court proceedings and assessments.
- Engage in ongoing professional development, including training and reflective practice.
- Stay updated on legislation, policies, and best practices in social care to inform decision-making.
Qualifications and Experience:
- Qualified Social Worker registered with Social Work England.
- Experience working in a social care setting, preferably within a locality or community-based team.
- Sound knowledge of relevant legislation, policies, and procedures governing social work practice, including the Children Act and Care Act.
- Strong assessment and case management skills, with the ability to formulate and implement effective care plans.
- Excellent communication and interpersonal skills, with the ability to build rapport and engage with diverse individuals and families.
- Ability to work collaboratively within multi-disciplinary teams and establish effective partnerships with external agencies and service providers.
- Commitment to promoting equality, diversity, and inclusion in service delivery, respecting the rights and cultural backgrounds of service users.
