Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time within an Adults Hospital Team in Newport.
The ideal candidate will possess highly developed specialist knowledge of social work practice in adults' services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £30 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Duties:
- Working collaboratively with health partners facilitating safe and timely discharges from hospital to the patient's next destination
- You will be required to work a rota covering Monday - Sunday and the pattern will be on a rota basis
- Ensure a high quality of practice that promotes independence, recovery, choice and autonomy
- To facilitate and support individuals to complete a needs assessment using a strengths-based approach, which focuses on the skills and abilities, considers their networks of support both within families and the wider community
Requirements of the Social Worker:
- Social Work England registration
- Degree or equivalent in social work
- Previous frontline social work experience within adult services
