Duties:

  • Liaise with colleagues in other departments/teams to ensure referral criteria and pathways are adhered to and to ensure information is shared appropriately with the relevant team/service in a timely manner
  • Make decisions in response to complex queries/referrals and evidence clearly the decision-making process with reference to legislation, policies and guidance
  • Make decisions in respect of safeguarding alerts/concerns and refer any appropriate alerts/concerns to the Adult Safeguarding Team for further enquiry/investigation
  • Provide constructive feedback where necessary to external organisations in respect of quality and appropriateness of information contained within referrals
  • Promote preventative approaches when appropriate
  • Participate fully in multi-agency risk management forums (MAPPA/MARAC) and take responsibility for the gathering and recording of relevant risk information on electronic systems
  • Undertake any training and development as necessary to meet the duties and responsibilities of the post
  • Attend and participate in relevant meetings with the aim of promoting effective joint-working and improving processes for the benefit of clients and professionals

Requirements of the Social Worker:

  • BA Social Work or equivalent, DipSW, CQSW, CSS
  • Registered with the Health and Care Professional Council
  • Be committed to safeguarding the residents of Stockton-on-Tees, ensuring that all safeguarding procedures are adhered to
  • Relevant experience and application of Legislation and Guidance
  • Experience of effective use of procedures and practice guidance
  • Experience of multi-disciplinary working
  • Experience of individual assessments of need
  • Experience of undertaking capacity assessments
  • Experience of care management and personalisation agenda
  • Experience of developing packages of care
