Sanctuary Health is currently looking for an experienced and dedicated Band 7 Senior Adolescent Outreach Case Worker to work full-time in South West London. The ideal candidate will possess highly developed specialist knowledge of mental health nursing and be registered with the NMC.

The pay rate for this locum Senior Adolescent Outreach Case Worker role is £32.91 per hour (Assignment rate) and is a 3-month contract.

Duties:

  • Act as a Care Co-ordinator within AOT, delivering timely and accessible assessment and treatment to young people aged 12-18
  • Work with these highly complex young people and their families, making appropriate useful formulations and providing interventions or making recommendations
  • Effective management of these high-risk cases within the community
  • Work closely with inpatient colleagues to ensure that the length of any admission is kept to a minimum
  • Participate in the referral and allocation process
  • Attend the Team's Care Plan Review Meetings
  • Plan and prioritise own workload
  • Maintaining high standards in clinical record keeping, including using computer-based data entry systems
  • Contribute to pre-and post-qualification teaching

Requirements of the Senior Adolescent Outreach Case Worker:

  • Recent experience within a similar role
  • Masters Degree in the relevant clinical subject or equivalent experience
  • Current professional registration in relevant clinical subject/practice e.g. RMN, OT, Social Work, Clinical Psychologist
  • Evidence of continuing Professional Development
  • Mentorship qualification or equivalent
  • Minimum of two years post-registration experience, including post-qualification and registration experience of working with young people
  • Experience of working in a multidisciplinary team
  • Knowledge of developmental models of mental health
  • Knowledge of psychotropic medications, medication management and monitoring and awareness of medication effects and side effects
