Sanctuary Health is currently looking for an experienced and dedicated Reablement Occupational therapist to work in Slough. The ideal candidate will possess highly developed specialist knowledge of Occupational Therapy and be registered with the HCPC.
The pay rate for this locum Occupational Therapist job is advertised at £29.78 (Assignment rate) and is an ongoing contract.
Duties:
- To work as an integral member of the Integrated Decisions making model. To be based in the locality and to provide rapid O.T assessments following a referral from the Cold Car paramedic or GP.
- The O.T will use their experience and expertise to clinically reason if service users can be supported by the integrated care and the other available community resources, to prevent admission by providing equipment and alternative compensatory approaches to activities of daily living.
- The post holder will assess and provide an O.T plan, which will help to prevent any unnecessary admissions or any further deterioration in the residents' abilities to functional at home.
- This will form part of a multi-professional approach enabling residents to achieve their maximum independence in their own home.
- The OT will provide an action plans to facilitate the involvement of the Reablement team, in appropriate cases, initiate equipment referrals to NRS to provide assistive equipment or where the Resident has a long-term package of care negotiate a temporary increase to ensure the safety of service user, with the relevant locality.
Requirements of the Occupational Therapist:
- Degree or equivalent within Occupational Therapy
- HCPC registered as an Occupational Therapist
- Recent NHS experience either pre or post-qualification
- Must be a car driver
