Sanctuary Health is currently looking for an experienced and dedicated Band 6 Discharge to Assess Physiotherapist to work full or part-time in across Buckinghamshire. The ideal candidate will possess highly developed specialist knowledge of physiotherapy and be registered with the HCPC.
The pay rate for this locum Physiotherapist job is £25.23 per hour (Assignment rate) and is a contract that lasts till the end of March 2021. It is a county wide community service - office base will depend where person lives.
Duties:
- Work as part of MDT team for patients in D2A care home beds across the county of Buckinghamshire with the OT & social worker.
- Assess the patient in the care home for therapy needs and functional requirements, care needs and have close liaisons with the social worker in order to agree discharge plans for the patient to be discharged home.
- Undertake follow up checks at home if required and if the patient has ongoing rehab needs, they will refer to the local community physio service unless the patient's progression/rehab/back to baseline can be completed in a few visits.
Requirements:
- Degree or equivalent within Physiotherapy
- HCPC registered as a Physiotherapist
- Recent post qualifying experience working as a Physiotherapist
- Previous NHS experience
- Use of Rio is desirable but not essential
- They need to be a car driver and have their own vehicle
