Main Duties:
- Develop and implement structured medication review format.
- Conduct clinical level 3 medication reviews for intermediate care unit patients and as requested by Case Managers.
- Assist in identifying patients needing pharmaceutical intervention.
- Generate medication review reports to optimise prescribing quality and cost-effectiveness.
- Implement agreed-upon changes based on review findings.
- Provide medication information, counseling, and training to patients, carers, and staff.
- Document medication review outcomes and conduct audits as needed.
- Potentially undertake Non-Medical Prescribing.
- Collaborate with healthcare professionals and social services for smooth medication implementation.
- Provide pharmaceutical support for palliative and end-of-life care.
- Offer pharmaceutical input for Expert Patient and disease-specific programs.
- Conduct training sessions on drugs and pharmacy topics for clinical staff.
- Utilise ICU computer system extensively for pharmacy operations.
- Ensure pharmacy supplies provision and monitor service level agreements.
- Plan, prioritise, and organise pharmacy service provision effectively.
Requirements of the Pharmacist:
- GPHC Registration
- Previous experience within a UK based NHS Pharmacy setting
- NVQ or equivalent – Pharmacist qualification
