Pharmacist Pay Rate: £33 - £36 per hour (assignment rate)

Main Duties:

  • Develop and implement structured medication review format.
  • Conduct clinical level 3 medication reviews for intermediate care unit patients and as requested by Case Managers.
  • Assist in identifying patients needing pharmaceutical intervention.
  • Generate medication review reports to optimise prescribing quality and cost-effectiveness.
  • Implement agreed-upon changes based on review findings.
  • Provide medication information, counseling, and training to patients, carers, and staff.
  • Document medication review outcomes and conduct audits as needed.
  • Potentially undertake Non-Medical Prescribing.
  • Collaborate with healthcare professionals and social services for smooth medication implementation.
  • Provide pharmaceutical support for palliative and end-of-life care.
  • Offer pharmaceutical input for Expert Patient and disease-specific programs.
  • Conduct training sessions on drugs and pharmacy topics for clinical staff.
  • Utilise ICU computer system extensively for pharmacy operations.
  • Ensure pharmacy supplies provision and monitor service level agreements.
  • Plan, prioritise, and organise pharmacy service provision effectively.

 

Requirements of the Pharmacist:

  • GPHC Registration
  • Previous experience within a UK based NHS Pharmacy setting
  • NVQ or equivalent – Pharmacist qualification
