Sanctuary Health is currently looking for an experienced and dedicated Band 6 Occupational Therapist to work 3.5 days per week within a Rapid Discharge/Home First Community Team based in Ormskirk. The ideal candidate will possess highly developed specialist knowledge of Occupational Therapy and be registered with the HCPC.
The pay rate for this locum Occupational Therapist job is £25 per hour (Assignment rate) and is a 4-month contract.
Main Duties:
- Assess patients at home within hours of coming being discharged from hospital. The care plan will include rehabilitation goals, support for carers, and any equipment which may be needed along with self-help advice.
- Rapid Response referrals into community services.
- Work with our community services Falls/Adult Rehab/Intermediate Care Beds
Requirements of the Occupational Therapist:
- Degree or equivalent within Occupational Therapy
- HCPC registered as an Occupational Therapist
- Recent NHS experience either pre or post qualification
- Must be a car driver
