Duties:
- Work flexible hours to meet the needs of patients and their families
- Provide high quality patient-centered care and specialist rehabilitation to adults referred to the Rehabilitation Service
- Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments
- Provide advice regarding the rehabilitation management of patients to families, carers and other members of the MDT
- Responsibility for a caseload of patients within both the community and / or the community hospital bedded environment
- Carry out duties autonomously but with the ability to seek support and informal and formal supervision from Senior Therapists within the teams
- Give support, guidance and training to Senior Rehabilitation Assistants, Rehabilitation Assistants and students on placement within the team
- Be responsible for organising, prioritising and planning your own caseload to meet service and patient priorities, readjusting plans as situations change/arise
- Ensure that your own practice, and that of the staff under your supervision, meets the required professional standards of Physiotherapy
- Ensure your own ongoing personal/professional development
- Comply with the departmental policies and procedures
- Adhere to your code of conduct
Requirements of the Community Rehab Occupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent NHS experience
- Registered with the HCPC
- Have experience of working with Older adults
- Car driver
