Duties:

  • Work flexible hours to meet the needs of patients and their families
  • Provide high quality patient-centered care and specialist rehabilitation to adults referred to the Rehabilitation Service
  • Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments
  • Provide advice regarding the rehabilitation management of patients to families, carers and other members of the MDT
  • Responsibility for a caseload of patients within both the community and / or the community hospital bedded environment
  • Carry out duties autonomously but with the ability to seek support and informal and formal supervision from Senior Therapists within the teams
  • Give support, guidance and training to Senior Rehabilitation Assistants, Rehabilitation Assistants and students on placement within the team
  • Be responsible for organising, prioritising and planning your own caseload to meet service and patient priorities, readjusting plans as situations change/arise
  • Ensure that your own practice, and that of the staff under your supervision, meets the required professional standards of Physiotherapy
  • Ensure your own ongoing personal/professional development
  • Comply with the departmental policies and procedures
  • Adhere to your code of conduct

Requirements of the Community Rehab Occupational Therapist:

  • Degree or equivalent in Occupational Therapy
  • Recent NHS experience
  • Registered with the HCPC
  • Have experience of working with Older adults
  • Car driver
