Sanctuary Personnel is currently looking for an experienced Band 6 Community Mental Health Nurse (CPN) to take on a new ongoing contract role within the Memory Assessment Team.
Location: Wandsworth
Pay Rate: £30 per hour (assignment rate)
Contract Type: Locum, 3-month contract

In this role you will be able to work Monday - Friday in the time frames from 09:00 - 17:00 pm, giving you the very best of good work life balance. Currently, in line with guidelines, the role requires a combination of working at the team base and working remotely.
Duties:
- Provide high quality initial assessment and carry out investigations for adults with memory problems, leading to diagnosis
- Undertake a review role for a defined caseload of clients with memory problems
- Develop a dedicated collaborative care plan that includes signposting to the appropriate support services and agencies for the clients and carers to enable them plan the future and responsible for reviewing this in accordance with the service spec
- Provide support and counselling to clients confirmed with diagnosis of dementia and their carer
Requirements of the Community Mental Health Nurse:
- Degree or equivalent in Mental Health Nursing
- Experience working as a Mental Health Nurse
- NMC Registration
- A highly motivated and experienced Band 6 Memory Assessment Nurse, with a passion to care for people with Dementia
- Have excellent documentation skills and be able to manage their time effectively
- Be able to manage medication in accordance with the Policy and the NMC Standards for administration of drugs
- Be conversant in and contribute to local initiatives on Clinical Governance, ie; Risk Management, Clinical Effectiveness and Local Clinical improvement Groups
- Be fully conversant with legislation relevant to current practice
