Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Mental Health Discharge Co-Ordinator to work full time based in Wyre Forest, Worcestershire.

The salary for this permanent Mental Health Discharge Co-Ordinator job is up to £44,496 per annum.

Main duties:

  • To be the lead professional on Safeguarding/Adult Protection matters within the team, ensuring that all team members are working in accordance with current safeguarding and adult protection policies and procedures and have received relevant safeguarding training for their role.
  • To chair Adult Protection meeting and attend these on behalf of the team where they are chaired by the Adult protection Team or other local authorities.
  • To act as a consultant to the team providing expert advice and guidance at the request or direction of the Team Manager.
  • To link with health staff and those other organisations to provide a coherent service to vulnerable adults.
  • To work alongside the Team Manager to ensure the smooth day-to-day operation of the Team, e.g. allocation and progression of work.
  • To deputise for the Area Manager as required.
  • To carry out staff appraisals as required, in accordance with agreed policy and to identify the learning and development needs of staff.
  • To assist the Team Manager in respect of ensuring safe and robust recording systems within the team and to monitor/audit case records on a regular basis.
  • To maintain personal and professional development to meet the changing demands of the job, participate in appropriate training activities and encourage and support staff in their development and training.
  • To undertake other such duties, training and/or hours of work as may be reasonably required and which are consistent with the general level of responsibility of this job.

  • Working with some vulnerable children, young people and adults can, at times, be emotionally challenging for which appropriate support will be provided through management supervision. The post holder must be able to deal with such mental demands.

Requirements of this role:

  • A social Work Qualification or equivalent.
  • Understanding of relevant legislation.
  • Registered or eligible for registration.
  • This post requires registration with the relevant health professional council/governing body (Social Care England).
