Sanctuary Social Care is currently looking for an experienced and dedicated Hospital Discharge Team Manager to fill a full-time role within an Adults' Services based in Middlesbrough.
The pay rate for this locum Hospital Discharge Team Manager job is £40.00 per hour (Assignment rate) and is an initial 6-month contract.
Duties:
- Project managing
- Supporting the Review Team with quality assurance
- Support customers who have been discharged into the community, residential and nursing homes
- Completing Care Act Assessments
- Long planning of care and support needs
- Managing a small team of social workers
Requirements of the Hospital Discharge Team Manager:
- A recognised and relevant professional social work qualification (Degree / Diploma in Social Work)
- Registered on Social Work England
- Previous experience as a manager in Hospital and Review Teams
- Knowledge and experience of working in hospitals
- Good knowledge of the Care Act 2014, Mental Capacity Act 2005 and Mental Capacity (Amendment) Act 2019 - Liberty Protection Safeguards
- Third-party DBS or update service
- Minimum of 4 years PQE
